Respecto a los trastornos de alimentación, algunos estudios (Bulik y col., 1997) investigan los antecedentes de trastornos de ansiedad en la infancia de mujeres con anorexia nerviosa (n=68) y bulimia nerviosa (n=116). Se hallaron con una frecuencia elevada, en el 90% de las anorexias y 94% de las bulímicas. La ansiedad social se asociaba significativamente a la bulimia, precediéndola, y con menor frecuencia a la anorexia nerviosa.

Asimismo la ansiedad social se ha asociado a trastornos dismórficos corporales – los cuales se inician con mayor frecuencia en la adolescencia -, sobre todo en mujeres (Veale y col., 1996). Algunos autores postulan que este trastorno y la fobia social pueden compartir factores etiológicos (Brawman-Mintzer y col., 1995) al observar la asociación de ambos en el 11% de su muestra de 54 pacientes.

La fobia social es uno de los trastornos a los que se asocia más psicopatología tanto del eje I como del II, agravando la clínica y complicando el tratamiento. El hecho de que la ansiedad social sea con frecuencia el trastorno primario, que originaría los demás, insta a un reconocimiento clínico y tratamiento precoz. La comorbilidad entre dos o más trastornos agrava la clínica, aumenta el deterioro académico y laboral e influye negativamente en la evolución clínica y respuesta al tratamiento.
